North West Boroughs Healthcare NHS Foundation Trust (formerly known as 5 Boroughs Partnership NHS Foundation Trust until 1st April 2017), provides physical health, mental health and learning disability services across the North West region. North West Boroughs Healthcare NHS Foundation Trust Assessment Team are in a position to appoint three successful candidates (one substantive; one six-month fixed term contract).
The Assessment Service provides assessments and treatment for people who may need support within secondary mental health services. The Assessment function offers support and access to services as well as ensuring smooth pathways between primary care and secondary services. The Team will provide a service for adults with moderate to severe symptoms of mental illness/disorder. In addition to mental health and risk assessments, clients who do not meet the referral criteria may be given advice and/or signposted to alternative services most suited to their needs. It is therefore important to foster and maintain effective partnerships with services in the non-statutory sector, to ensure effective care for people with less severe conditions.
The service operates 24 hours per day, seven days per week.
This post is subject to an enhanced Disclosure and Barring Service (DBS) Check. Successful candidates will either have a valid DBS check with the Trust or will be required to pay for the cost of the DBS Disclosure which is £47.50 for an enhanced disclosure, the cost of which will be deducted from first salary.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
