An opportunity has arisen to join the community nursing team within Knowsley. The post holder will work as part of the newly created unscheduled care team. This team receives and co-ordinates all new referrals into district nursing teams, provides a point for clinical triage and responsive intervention and liaises with the wider district nursing teams and other services to ensure a safe transfer of care for patients .
You will have the opportunity to influence and shape the development of this team and provide safe, effective , high quality care to our patients.
We are interested in candidates who have a passion for care delivery and the development of staff and who can demonstrate high levels of organisation and risk management.
Community Nursing is a 7 day service which operates between 8am-8pm and the post holder must be able to work within this framework.

This post is subject to an enhanced Disclosure and Barring Service (DBS) Check. Successful candidates will either have a valid DBS check with the Trust or will be required to pay for the cost of the DBS Disclosure which is £47.50 for an enhanced disclosure, the cost of which will be deducted from first salary.
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
