An exciting opportunity has arisen within the Knowsley Assessment and HTT for an experienced Practitioner to join a dynamic Mental Health Community Team.
The role involves working in response to front-line mental health demands for adult registered with a Knowsley G.P. The core roles include; telephone triage assessment, specialist mental health assessments, deliver of home treatment interventions and duty co-ordination. The role also involves bed management and 136 co-ordination.
Access to transport for the post is essential due to the majority of the interventions taking place in the community setting.
The Assessment Teams core operational Hours are between 8am – 8pm Monday to Sunday with out of hours cover 8pm – 8am, in the form of a waking night shift covered by Senior Nurse Practitioners from within the teams who will respond to emergency referrals from A&E, 136 sections and out of hours GPs. The post holder will demonstrate an attitude which respects and values service users’ choice and works alongside them and their carers to ensure they are at the centre of care planning.
The successful candidate must have excellent communication skills where they can work effectively with multi-disciplinary teams and professionals from similar backgrounds as well as clients and their families / carers. They should be able to work independently as well as part of a closely supported team and have the ability to prioritize their work load effectively. This post is subject to an enhanced Disclosure and Barring Service (DBS) Check. Successful candidates will either have a valid DBS check with the Trust or will be required to pay for the cost of the DBS Disclosure which is £47.50 for an enhanced disclosure , the cost of which will be deducted from first salary.
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
