An opportunity has arisen for a new post as a Discharge Co-ordinator. The post is to primarily work across the Whiston Hospital and Northwest Boroughs Healthcare Trust in conjunction with the Liaison service to identify and ensure complex discharges and transfers of a mental health nature are managed efficiently and effectively to avoid delays. Also to establish patient pathways between primary, secondary and community services to expedite timely discharge and improve patient flow throughout.
They will act as a specialist link between Trusts and key stakeholders to promote collaborative working and provide a seamless service for patients. In conjunction with the MDT ensure that patients have individualised discharge plans of care that are relevant for their needs in the community.
They will be responsible for the continued development and promotion of a patient focused discharge process for patient discharges within the Acute Trust, and Advise and support staff in identifying issues relating to the effective discharge of patients from hospital. The successful person will be pivotal in managing the pathways between the Home Treatment Team and mental health inpatient wards.

This post is subject to an enhanced Disclosure and Barring Service (DBS) Check. Successful candidates will either have a valid DBS check with the Trust or will be required to pay for the cost of the DBS Disclosure which is £47.50 for an enhanced disclosure and £29.50 for a standard disclosure, the cost of which will be deducted from first salary.
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
