An exciting opportunity has arisen for a committed and experienced member of staff with appropriate clinical skills in assessing  elderly patients using a multi-factorial assessment tool that follows NICE guidance for both falls and fracture risk and who can bring enthusiasm and energy to the Falls assessor role.

The post holder will be responsible for following clinical assessment processes across the community based Falls, Wellbeing and Postural Stability Services, including assessment, recommendations and evaluation of the most complex patients within the service.

The Post holder will show the ability to work to a high standard, working both independently and within a multi-skilled team and demonstrate the willingness to both learn, adapt and use new skills, They will show the clear ability to rationalise and discuss clinical findings with the clinical leads within the service where appropriate whilst also showing the ability to work independently and safely.

 The post holder will promote sharing of best practice and consistency across the whole of the service and work with the senior leadership team to implement service developments to meet national and local healthcare agendas.

If you have the drive, commitment and enthusiasm and relevant experience to successfully join the falls and wellbeing service across the integrated community and acute patient pathways in Knowsley and are able to rise to the challenge then we want to hear from you.

The post holder will be experienced in assessing physical health  with specialist falls and fracture risk knowledge, significant care of the elderly experience and a proven track record working in a Community Nursing Team.

This post is subject to an enhanced Disclosure and Barring Service (DBS) Check. Successful candidates will either have a valid DBS check with the Trust or will be required to pay for the cost of the DBS Disclosure which is £47.50 for an enhanced disclosure, the cost of which will be deducted from first salary.
